CenterWell Home Health seeks a compassionate Home Health Occupational Therapy Assistant to deliver in-home therapeutic care that maximizes patient independence and safety. Working under an Occupational Therapist, you'll provide ADL training, home safety assessments, and caregiver education while following individualized care plans. You'll document progress in EMR, collaborate with an interdisciplinary team, and support CenterWell's culture of compassion, integrity, and clinical excellence in every patient visit.
