This position manages the full admissions process, collaborating with referral sources, physicians, case managers, and families to ensure each patient's transition to hospice care is handled with urgency, accuracy, and empathy. You will communicate by phone and in the field, coordinating services, verifying insurance, and ensuring documentation compliance - all while supporting the mission to provide high-quality, patient-centered care.
