Aya Healthcare - - Responsibilities: Oversee the referral and admission process for the Inpatient Rehabilitation Center in conjunction with unit leadership and the Medical Director; Complete the pre-admission assessment of candidates referred by acute care case managers, therapists, physicians, or external referral sources; Determine patient/family overall goal for rehabilitation and preliminary discharge plan; Seeks pre-authorization from insurance companies and communicates availability of rehab benefits to prospective admissions; Educate referral sources and staff related to admission criteria and regulatory changes that affect the rehab level of care
