The employee will be providing social work professional services to clients with HIV/ AIDS who have complex medical, behavioral, and social needs at the East Carolina HIV program in the Division of Infectious Disease. The employee will provide direct supervision of HIV Medical Case Managers; knowledge of the Ryan White Program; review and audit client charts to ensure accuracy, completeness, and compliance with Ryan White Program standards; provide consultation on challenging client situations; assist with crisis intervention and coordination of care as needed; coordinate with internal departments (clinical, behavioral health, pharmacy, etc.) to ensure integrated care; ensure accurate and timely documentation in Electronic Health Records (EHR) and any required data systems (e.g. CARE - Ware); co-facilitate and participate in the Region 10 Network quarterly meetings; establish the Annual Region 10 Client Satisfactory Services; coordinate the HIV partner testing program; participate in t...Behavioral Health, Practitioner, Journey, Healthcare, Program
